✔️ Words of Advice:

• Do not take Acepril 10 Tablet if you are allergic to Acepril 10 Tablet or any of its contents.
• Talk to your doctor if you have heart, kidney, or liver problems.
• Acepril 10 Tablet may sometimes cause lower blood pressure than normal, especially in dehydrated people who take diuretics (medicines to treat oedema) and have low sodium levels. Regularly check your blood pressure levels to avoid any risks.
• Avoid taking Acepril 10 Tablet if you are pregnant or breastfeeding.
• Acepril 10 Tablet is not recommended for children below 18 years of age, as efficacy and safety have not been established.
• Consult your doctor if you have lost a lot of body fluids through vomiting, diarrhoea, or sweating more than usual, or if you are taking water tablets to treat oedema (diuretics), or if you undergo dialysis.

✔️ Dosage & Administration

Hypertension (High Blood Pressure):
The usual starting dose is 10 mg once daily. The first dose is preferably taken at bedtime to reduce the risk of a sudden drop in blood pressure.

• In patients with renovascular hypertension, fluid depletion, or severe hypertension: start with 2.5–5 mg once daily.
• In patients already taking diuretics: start with 5 mg once daily.
• Maintenance dose is typically 20 mg once daily and may be increased up to 80 mg/day depending on response.

Heart Failure:
Used as an add-on therapy. Start with 2.5–5 mg once daily and increase gradually (by more than 10 mg at intervals of at least 2 weeks) up to a maximum maintenance dose of 40 mg/day.

Post-Myocardial Infarction (Heart Attack):
Treatment should begin within 24 hours of symptom onset. Start with 5 mg once daily for the first 2 days, then increase to 10 mg once daily.

• In patients with low systolic blood pressure: start with 2.5 mg once daily.

Diabetic Nephropathy:
For hypertensive patients with type 2 diabetes and microalbuminuria, the usual starting dose is 10 mg once daily, which may be increased to 20 mg once daily to achieve a diastolic blood pressure below 90 mmHg.

Child Dose

Hypertension (Children >6 years):
Initial dose is 0.07 mg/kg once daily, up to a maximum of 5 mg/day.

Renal Dose Adjustment

Adults with Renal Impairment:

• CrCl <10 mL/min or on dialysis: start with 2.5 mg once daily
• CrCl 10–30 mL/min: start with 2.5–5 mg once daily
• CrCl 31–80 mL/min: start with 5–10 mg once daily
  Dose may be adjusted up to a maximum of 40 mg/day based on patient response.

Children:

• Not recommended if GFR <30 mL/min/1.73 m²
• CrCl <10 mL/min: 2.5 mg once daily
• CrCl 10–30 mL/min: 2.5–5 mg once daily
• CrCl 31–80 mL/min: 5–10 mg once daily

✔️ Contraindications:

• History of angioedema related to ACE inhibitors
• Hereditary or idiopathic angioedema
• Bilateral renal artery stenosis
• Pregnancy (second and third trimester)
• Breastfeeding

✔️ Precautions & Warnings

• Use with caution in patients with low blood volume (hypovolaemia) or high potassium levels (hyperkalaemia)
• Careful use in collagen vascular diseases, valvular heart disease, or renal artery stenosis
• Monitor renal function before and during treatment
• May cause dizziness; caution when driving or operating machinery
• Not recommended in children under 6 years
• Use during breastfeeding is not advised due to lack of safety data
